I, the parent or legal guardian of the child listed on this form, certify that he/she has my full approval to participate in the programs of Sunset Bible Camp. The child identified on this form understands that he/she is expected to abide by Sunset Bible Camp guidelines and be directly responsible to the Dean in charge. Sunset Bible Camp assumes responsibility for discipline at all of its programs and events and, if necessary, may, because of misconduct or disobedience, require a student to leave, in such instance, I will assume full responsibility for returning my child home.

In case of emergency: I hereby give my permission to the physician selected by the camp management, to hospitalize, secure treatment for, and to order injection, anesthesia, or surgery for my child as named on this form. I understand however that every effort will be made to contact me in case of such an emergency and, if possible before any such medical treatment is administered. I hereby release the Camp from any responsibility other than normal supervision and care. In case of an accident, I will not hold Sunset Bible Camp or its staff members, management, or officers liable unless guilty of negligence.
